Equine Insurance Application

Equine Mortality coverage, including options for medical/surgical, can be completed by the owner or an agent who is most familiar with the horse.

Veterinary

When requested, this form should be completed by a veterinarian familiar with the horse (especially if pertaining to past health or medical issues). Required for all horses valued at $100,000+ along with the Equine Insurance Application.

Examination Form

This form should be completed by your horse’s veteranarian.

Declaration of Health Form

When requested, this form should be completed by the owner or person responsible for the horse(s).

Health History Questionnaire

When requested, this form can be completed by the seller or trainer who is most familiar with the horse’s health history. Submit with the Equine Insurance Application.

Health Statement Form

When requested, this form states the overall health and soundness of a horse. To be completed by the Owner or Agent who is familiar with the horse. Submit with the Equine Insurance Application.

Loss Of Use Examination Form

Loss of Use applicants must submit the following paperwork from their Vet: Loss of Use Exam along with stated Radiographs, the Veterinary Examination form (above), and an Equine Insurance Application completed by the Owner. We may also require acknowledgement forms regarding this coverage.

Appraisal Form

When requested, the appraisal form is available for a professional trainer to help establish the current fair market value of a horse. This must be submitted with the Equine Insurance application. Established values are subject to company approval.
